Start from a page that can hold a form
If your workspace has no page yet, create a funnel from a blank page or from a template that includes a form. If you use the AI page generator, treat its output as a draft: it invents testimonials and adds no form. Delete any review or testimonial you did not collect, then add your own form.

In Sites, open the website or funnel page you want to work on. Find Forms & Surveys in the builder sidebar.

Add a focused form
Choose a new or existing form. A new form starts with two consent checkboxes and placeholder business text; remove or rewrite them and keep only one that matches how you will actually contact people. Keep the fields relevant to the inquiry, style the button, and set a helpful confirmation message or destination.

Preview the page and test your form
Save the page and use its preview to submit your test details. Check the entry in the Forms submissions area before sharing the page.

Steps:
